The Native American Elders Outreach Project
The creation of the Wisconsin Native American Elders Resource Directory 2000 was funded by Wisconsin Judicare, Inc.’s Native American Elders Outreach Project (NAEOP). The NAEOP began in March 1993 with a grant from the Wisconsin Department of Health and Family Services using federal Health Care Financing Administration funds. Over the last seven years, one of the primary components of the project has been presentations to Native American elders at tribal nutrition sites. These presentations have focused on health care and income maintenance programs. Specific topics have included, for example, "Medicare, Medicaid, and Indian Health Service: How Do They Relate to Each Other?" and "The Effect of Income Unique to Native American Elders on Public Benefit Eligibility.”
Before and after presentations to elders, a staff attorney from Wisconsin Judicare is available to meet with individual elders to answer legal questions and, in some cases, provide legal representation or a referral to another attorney. Part of the NAEOP grant funds have also been used to create videos and booklets for Native American Elders, such as this Directory. Additionally, the NAEOP has organized training sessions for lay advocates, tribal benefit specialists, county benefit specialists and tribal aging unit directors.
Native American elders who live in Wisconsin who have questions on health and income maintenance topics such as Medicare, Medical Assistance (Medicaid), the Indian Health Service, Social Security, Supplemental Security Income (SSI), elder abuse, powers of attorney for health care and living wills should contact Wisconsin Judicare for assistance. There is no cost to elders who participate in the project and all Native American elders in Wisconsin are eligible regardless of income. Section One
Summary of Benefits for Native American Elders
This Section of the Directory provides brief descriptions of numerous programs of interest to Native American elders. For more information on any of the following programs, elders should contact the governmental agency or program directly. The advocates described under the General Benefits Assistance heading and the Legal Assistance heading can also assist elders. Local telephone numbers for agencies such as Tribal Social Services are listed by Tribe in Section Two of this Directory. Section Three also contains many important telephone numbers, such as the nationwide Social Security and Railroad Retirement Board telephone numbers.
General Benefits Assistance
Tribal Benefit Specialists and County Benefit Specialists. Each Tribe and County has a benefit specialist who can explain benefit programs, assist elders with applications for benefits and refer elders to other agencies that can provide assistance. The services provided by a benefit specialist are free.
Medical Benefit Specialist. Many Tribes have a Medical Benefit Specialist at their health care centers to assist with applications for Medical Assistance. Contact your Tribal health care center for more information.
Native American Elders Outreach Project (NAEOP) staff attorney. Wisconsin Judicare’s Native American Elders Outreach Project has a staff attorney available to work with Native American elders who live anywhere in Wisconsin. This attorney can provide information on benefits and, in some case, represent elders in appeals and other matters. For more information, contact Wisconsin Judicare, Inc. at 1-800-472-1638 and ask for the NAEOP staff attorney.
Tribal and County Aging Units/Departments on Aging. Each Tribe and County has a department or unit that is dedicated to providing services to elders. Services provided can include home-delivered meals, chore services, exercise programs and other services. For more information on the services available in your area, contact the Tribal or County Aging Unit.
First Call for Help. Many counties have a free telephone hotline that can assist elders in finding telephone numbers in their area. To find a local number for this service, look under First Call for Help in the telephone book.
Health Care Financing
Medicare. Medicare provides health care insurance at age 65 to those who are insured through the Social Security program. Certain disabled persons are also eligible for Medicare. There is no monthly premium for Part A, but there is a monthly premium for Part B ($45.50 per month for 2000). To apply for Medicare, contact the Social Security Administration, either through the national 800 number, 1-800-772-1213, or by contacting a local Social Security office. Note: Railroad Retirement Beneficiaries should contact the Railroad Retirement Board to enroll in Medicare.
Medicare Supplement Plans. Individuals can purchase additional private health insurance to supplement their Medicare plan. For information on Medicare Supplement Plans, contact the State of Wisconsin Office of the Commissioner of Insurance at 1-800-236-8517 or the Medigap Helpine at 1-800-242-1060.
Medical Assistance (Medicaid). This program provides health insurance to low-income, low-asset individuals who are disabled or at least age 65. Some caretakers of minor children also receive Medical Assistance. In Wisconsin, Supplemental Security Income (SSI) recipients automatically receive Medical Assistance. Others can apply for Medical Assistance at their Tribal or County Social Services Office.
QMB, SLMB, and SLMB+. These programs pay some of the unreimbursed costs associated with Medicare, such as the Part B Medicare Premium of $45.50 a month, for Medicare recipients who meet income and asset guidelines. The financial limits are higher than the limits for Medical Assistance, so it can be easier to qualify for these programs. To apply, applicants should contact their Tribal or County Social Services Office.
Indian Health Service. All Tribes in Wisconsin have health clinics that are funded in part by federal dollars from the Indian Health Service. In addition to providing health care on site, the clinics provide vouchers through the Contract Health Service program for Native Americans to obtain treatment at non-Indian facilities. For information on eligibility, contact the Tribal clinic where you live. You can also contact Contract Health Service Specialist Gretchen Haug at Great Lakes Inter-Tribal Council, 1-800-472-7207.
United States Department of Veterans Affairs Health Program. The U.S. Department of Veterans Affairs provides health benefits to veterans at a variety of health centers throughout the state. Veterans can contact their Tribal or County Veterans Service Officer for more information. Veterans can also call (toll-free) 1-877-222-8387 to find out more about the health care program.
WisconCare. This health care program is operated by the State of Wisconsin and provides health care to a limited number of applicants each year. It is available to residents of the following Wisconsin counties: Brown, Door, Douglas, Forest, Kenosha, Lincoln, Milwaukee, Oconto, Oneida, Portage, Price, Sawyer, Shawano, Vilas, Washburn, Waukesha and Waushara. Hospital Patient Assistance Programs. Many hospitals and medical centers have programs that pay some or all health care costs for those with limited resources who earn too much to qualify for Medical Assistance. For more information, contact the medical facility and ask for information on patient financial assistance programs.
Patient Assistance Program (Indigent Drug Program). Low-income patients who need assistance paying for prescription drugs may be eligible to receive free prescription drugs from drug manufacturers. For more information, ask your doctor about this program or contact a Tribal or County Benefit Specialist. You can also download a free directory of program participants from
Donated Dental Services. The Wisconsin Dental Association provides a low-cost program to disabled, critically ill or elderly patients. For more information contact the Wisconsin Dental Association at 1-888-338-6852.
Long-Term Care
Nursing Home Care. Numerous nursing homes and skilled nursing facilities provide services to elders in need of special care. For information on nursing homes and other facilities in your area, talk to your doctor or hospital social worker. The hospital social worker, a benefit specialist or an attorney can also provide you with information on financing nursing home care privately or through the State of Wisconsin’s Medical Assistance Nursing Home program. Residents of nursing homes who experience problems in the facility are encouraged to contact a Long-Term Care Ombudsman at 1-800-815-0015.
Home and Community-Based Programs. The State of Wisconsin has several programs that provide home-based services to at-risk individuals so that they can remain in their homes instead of entering a nursing home or other long-term care facility. These programs include the Community Options Program (COP), COP-Medical Assistance Waiver (COP Waiver), Community Integration Program (CIP II), and Family Care. Funds for these programs are limited, so some applicants are put on waiting lists. For information on these programs, contact Tribal or County Social Services.
Coalition of Wisconsin Aging Groups (CWAG) Guardianship Support Center. The Guardianship Support Center provides free information to elders and their families on issues such as guardianship, nursing homes, powers of attorney for health care, powers of attorney for finances, and other matters. Financial Benefits
Social Security Retirement, Survivors and Disability Benefits. Many workers and their families qualify for Social Security benefits at some point in their lives. For information on these benefits, contact the Social Security Administration at 1-800-772-1213, or contact a local Social Security office.
Supplemental Security Income (SSI). This program provides a financial benefit and free Medical Assistance to low-income, low-asset individuals who are disabled or at least age 65. Federal SSI recipients in Wisconsin also automatically receive Medical Assistance and a State SSI supplement each month. For information on SSI, contact the Social Security Administration at 1-800-772-1213, or contact a local Social Security office.
State Supplemental Security Income (SSI). All Federal SSI recipients in Wisconsin automatically receive a State SSI supplement. Additionally, there are a small number of people who receive only a State SSI supplement. Recipients of the State SSI supplement can contact the State of Wisconsin’s SSI program at 1-800-675-0249 if they have any problems with their State SSI payments.
Veterans Benefits. Both the United States Department of Veterans Affairs and the Wisconsin Department of Veterans Affairs offer a variety of benefits to veterans. For information on these programs, contact a Tribal or County Veterans Service Officer; the United States Department of Veterans Affairs, Milwaukee Regional Office, at 1-800-827-1000; or the Wisconsin Department of Veterans Affairs at 1-800-947-8387.
Railroad Retirement. Railroad employees and their families may be eligible for benefits through the Railroad Retirement Board. Recipients of Railroad Retirement benefits also receive Medicare through the Railroad Retirement Board. Kinship Care. Elders who are caring for minor children may be eligible for financial assistance. For more information contact Tribal or County Social Services.
Food Stamps and Commodities. Food Stamps or distributions of food baskets are available to low income, low-asset individuals. For more information, contact Tribal or County Social Services.
Energy and Weatherization Assistance. These programs provide assistance to low-income individuals. Different agencies administer these programs throughout the state. To find out how to apply in your area, contact your Tribal or County Social Services office, or a Benefit Specialist, and ask for information on the Low-Income Energy Assistance Program.
Disability
Aging and Disability Resource Centers. Pursuant to a new long-term care redesign program in Wisconsin, there are nine Aging and Disability Resource Centers in Wisconsin that provide information and assistance in accessing benefits and services. More centers may open in the future. The nine locations currently open are: Fond du Lac Department of Social Services, Jackson County Department of Health and Human Services, Kenosha County Division of Aging Services, LaCrosse County Human Services Department, Marathon County Commission on Aging, Milwaukee County Department on Aging, Portage County Department on Aging, Trempealeau County Department of Social Services and the Oneida Tribe’s Community Options Program.
Social Security Administration Programs. Disabled individuals can receive financial benefits from the Social Security Administration through the Social Security Disability Insurance (SSDI) program and/or the Supplemental Security Income (SSI) program. The Social Security Administration also has several programs designed to help disabled individuals return to work. Wisconsin Department of Vocational Rehabilitation (DVR). This agency provides assistance to disabled individuals who would like to return to the workforce. For more information, contact a local DVR office or contact DVR at 1-800-442-3477.
Wisconsin Coalition for Advocacy. This advocacy group assists disabled individuals and their families and can provide general information. Contact the Wisconsin Coalition for Advocacy at 1-800-928-8778.
Legal Assistance
Lawyer Referral and Information Service. The State Bar of Wisconsin provides a free referral service to callers seeking to hire an attorney in their area. Callers will be asked to briefly describe their legal problem. If the Service determines that an attorney consultation would be helpful, the Service will arrange for a free or reduced-fee half-hour consultation for the caller. The number for the Lawyer Referral and Information Service is 1-800-362-9082.
Legal Services Programs. There are four non-profit law firms in Wisconsin that are funded by the federal Legal Services Corporation. These programs provide free civil legal services to residents who meet certain income and asset guidelines. However, program funds are limited so not all cases are accepted. The four programs include: Legal Action of Wisconsin, Legal Services of Northeastern Wisconsin, Western Wisconsin Legal Services, and Wisconsin Judicare, Inc. There are also several other free civil legal services programs. Telephone numbers for many of the programs are listed in Section Three of this Directory, under Legal Services.
Power of Attorney for Health Care and Living Will Forms. The State of Wisconsin has created forms that allow a person to make decisions about his future health care that will direct doctors and others if the person is ever unable to communicate his health care wishes. The Power of Attorney for Health Care document is available by sending a self-addressed, business-size envelope to “Power of Attorney, Division of Health, P.O. Box 309, Madison, WI 53701-0309.” The Living Will is available by sending a self-addressed, business-size envelope to “Living Will, Division of Health, P.O. Power of Attorney for Finances Forms. The State of Wisconsin has created a form so that adults can designate an agent to perform financial tasks such as banking and real estate transactions. Often, it is best to consult an attorney before executing a Power of Attorney for Finances document. However, the state form is available on the internet at DHFS Advance Directives Also, the Coalition of Wisconsin Aging Groups has prepared a written Do-It-Yourself Consumer Kit that is available by calling CWAG at 1-800-488-2596. CWAG’s written instructions, but not the form, are also available on the internet at www.cwag.org.
Abuse
Domestic Abuse. Elders who experience Domestic Abuse or who have questions about Domestic Abuse are encouraged to contact local law enforcement, a local Domestic Abuse shelter, or the Wisconsin Coalition Against Domestic Violence at 1-715-849-3610 (Wausau) or 1-608-255-0539 (Madison).
Elder Abuse. All counties and the Oneida Tribe have a lead elder abuse agency that anyone can contact to report elder abuse. For more information, contact the lead elder abuse agency in your area. The lead agencies are listed by Tribe in Section Two of this Directory.
Taxes
Internal Revenue Service (IRS). The United States Internal Revenue Service collects federal taxes. For information on the IRS and federal taxes, contact the IRS at 1-800-829-1040.
Wisconsin Department of Revenue. The State of Wisconsin’s tax agency is called the Wisconsin Department of Revenue. Even those who do not pay state tax may be eligible for the Homestead Tax Credit program. For more information, contact the Wisconsin Department of Revenue at 1-608-266-2772.
Programs for Tribal Members
Bureau of Indian Affairs. This federal agency can provide information on a variety of benefits available to Native Americans. For more information, contact the regional office at 1-612-713-4400, or the Ashland field office at 1-715-682-4527.
Great Lakes Inter-Tribal Council, Inc. Contact this organization for information on a variety of programs, including the Foster Grandparent and Senior Companion Programs, the Traditional Foods program (providing wild game to elders), and others. The telephone number is 1-800-472-7207.
